The 2009 Kris Perani Hockey Foundation Sponsorship Application is now available. Please be advised that application dates have changed. Applications will be accepted May 1, 2009 through August 31st, 2009 for the 2009 – 2010 hockey season. All decisions regarding the applications will be made on September 15th, 2009 and monies will be mailed the first week of October, 2009.

If you do not have Adobe Acrobat Reader on your computer, you may download it for free from Adobe. For legibility, you may fill out of the form on your computer, then print the form which must be returned via postal mail with the rest of your application packet items as detailed on the application.

Mission Statement
The Kris Perani Hockey Foundation is an organization whose goal is to provide hockey players in need with financial assistance. The benefit of sponsoring hockey players in need will strengthen the hockey community and have a positive impact on the chosen nominees and their families. The Kris Perani Hockey Foundation is a public charity under section 501 (c) (3) of the Internal Revenue Code.
