The 2011 Kris Perani Hockey Foundation Sponsorship Application is now available. The application deadlines for the Kris Perani Hockey Foundation are as follows: - Deadline A: Applications will be accepted now through July 31st, 2011. This is for those individuals whose teams had tryouts before August, 2011. Decisions for this group will be made in August and notifications will go out at that time.
- Deadline B: Applications will be accepted from August 1st until September 30th, 2011. This is for those individuals whose teams are having tryouts in August and later in the fall. Decisions for this group will be made the first week in October and notifications will go out at that time.
All monies for those sponsored will be sent out directly to your team the second week of October, 2011.

If you do not have Adobe Acrobat Reader on your computer, you may download it for free from Adobe. For legibility, you may fill out of the form on your computer, then print the form which must be returned via postal mail with the rest of your application packet items as detailed on the application.

Mission Statement
The Kris Perani Hockey Foundation is an organization whose goal is to provide hockey players in need with financial assistance. The benefit of sponsoring hockey players in need will strengthen the hockey community and have a positive impact on the chosen nominees and their families. The Kris Perani Hockey Foundation is a public charity under section 501 (c) (3) of the Internal Revenue Code.
