Please enclose the following items with your application
1.  A 1,000 word essay on why the player should be sponsored
2.  Three (3) recommendation letters from the coach, the president of the association, and a team parent
3.  Copy of 2010 1040 Tax Return Form
4.  Ice bill for the year
